Acceptance of Terms
These Terms of Use govern your access to and use of the ProlongMD™ website and related services. By accessing this website or using ProlongMD™ services, you agree to these Terms. If you do not agree, do not use the website or services.
Eligibility
ProlongMD™ services are intended for individuals 18 years of age or older, or minors where permitted by law with the consent of a parent or legal guardian.
Cash-Pay Medical Practice
ProlongMD™ operates on a cash-pay only basis. We do not accept commercial insurance, Medicare, Medicaid, TRICARE, or other third-party payer programs. You agree not to submit claims for reimbursement to any insurer for services provided by ProlongMD™ or its Providers.
Medical Services and Provider Relationship
ProlongMD™ contracts with licensed physicians, nurse practitioners, and physician assistants (“Providers”) as independent contractors. Providers practice under their own professional licenses and applicable state laws. ProlongMD™ does not practice medicine and does not control or interfere with a Provider’s independent medical judgment.
Your provider-patient relationship is with your assigned Provider, not with ProlongMD™ as an entity.
Telehealth Services
Medical services may be provided through telehealth technologies, including video, phone, secure messaging, and other electronic communications. Telehealth has limitations and may not be appropriate for all conditions. Your Provider may determine that in-person evaluation or additional testing is required.
ProlongMD™ does not provide emergency medical services. In an emergency, call 911 or go to the nearest emergency department.
Pharmacies, Labs, and Third Parties
Prescriptions may be fulfilled by independent, licensed pharmacies. Laboratory testing may be coordinated through third-party laboratories. ProlongMD™ is not responsible for the services, errors, delays, or quality of care provided by pharmacies, laboratories, or other third parties.
Intellectual Property
All content on this website, including text, graphics, logos, and trademarks, is owned by or licensed to ProlongMD™ and may not be copied, modified, or used without permission except as permitted by law.
No Guarantee of Treatment or Results
Medical treatment decisions are made solely by your Provider. There is no guarantee that you will receive any particular medication, treatment, or outcome.
Dispute Resolution and Limitation of Liability
Any dispute arising out of or relating to your use of ProlongMD™ services will be resolved by binding arbitrationunder the rules of the American Arbitration Association. You waive the right to a jury trial and participation in class actions.
To the fullest extent permitted by law, ProlongMD™’s total liability for any claim related to services shall not exceed $1,000.
Changes to These Terms
We may update these Terms from time to time. Continued use of the website or services constitutes acceptance of the revised Terms.
