Telehealth Consent

By using ProlongMD™ services, you consent to receive healthcare via telehealth technologies, including video visits, phone calls, secure messaging, and electronic communications. Telehealth may not be appropriate for all conditions, and your Provider may require in-person care or additional testing.

Electronic Communications

You authorize ProlongMD™ and its Providers to communicate with you via secure messaging, text messages (SMS), email, and in-platform notifications for treatment-related, operational, and limited educational purposes. You may opt out of non-essential communications.

Electronic communications are not for emergencies.

Off-Label and Compounded Therapies

Your Provider may recommend or prescribe medications for off-label use or compounded formulations when medically appropriate. These therapies may not be FDA-approved for your specific indication. Risks, benefits, alternatives, and uncertainties are discussed as part of informed consent.

No Emergency Services

ProlongMD™ does not provide emergency care. If you experience a medical emergency, call 911 or seek immediate in-person medical attention.
